What's on the sheet
- Company, project or job site, and date at the top.
- Topic and presenter, so the sheet says what was covered and by whom.
- An attendance line workers sign under: “By signing, I confirm I attended this safety meeting and understood it.”
- 20 numbered rows for printed name, signature and company or trade.
Using it well
- Fill in the topic, date and presenter before the meeting, so the crew signs a complete record.
- Have each worker print and sign their own name. A list written by one person proves less.
- Use one sheet per meeting, and keep it with the talk you gave.
- Use the side-by-side version for a mixed crew, so everyone can read what they're signing.

Is it free?

Does OSHA require a toolbox talk sign-in sheet?
Federal OSHA doesn't publish a required form. It does require employers to train workers to recognize and avoid unsafe conditions (29 CFR 1926.21(b)(2)), and a signed sheet is the simplest record that a meeting happened and who was there. Some states go further: in California, crews must hold toolbox or tailgate safety meetings at least every 10 working days (Title 8, §1509(e)), and Cal/OSHA's guidance recommends recording each meeting's date, topic and attendees.
Can I use it for any topic?

Can workers sign on their phones instead?
