Each toolbox talk, JHA or safety plan you write is one document. Printing it, downloading the PDF again and collecting signatures don't count. A document that fails to write doesn't count either.

Yes. Your whole crew can sign every talk, however many workers you have. The plan only limits how many documents you write each month.
