Questions
How do I fill in a JHA?
Pick one task. Write its steps in the order the crew does them, from setup to cleanup. For each step, write what could hurt someone, then how you'll prevent it: remove the hazard if you can, then guards or barriers, then safe work practices, then PPE. Review it with the crew before work and have them sign.
Is this an OSHA form?
No. OSHA doesn't publish a required JHA form; its guide Job Hazard Analysis (OSHA 3071) describes the method and includes a sample. This template follows the same method: steps, hazards and controls.
